When:
After you see the intro for THE END, go back to area where the guards were, now save, if para-medic says sure want to save snake? then your in the wrong area, after you've saved exit the game.
How:
Take the game out of the DVD drive, and go to the system configuration and then set the clock a week or so foward, then start the game again and go to the area where you last met the end. The movie clip should come on and snake should go up to the end HE's DEAD. I won't spoil the movie bit and you may even get a secret item!
