How:
Unlock the Grim Reaper and unlock another character's (any) alternate costume (cannot be done with Clive Winston and Xavier Stone as they do not have an alternate outfit). Now select the Grim Reaper and then select another character. He will now play the Scythe (unless you manually change the guitar again).
